What Are the Most Popular Breast Implant Sizes Today? 

A Midwest Plastic Surgery Practice Shared Its Real Data

Breast augmentation is one of the most personalized surgical decisions a person can make. Yet, most of the “popular size” content on the internet is based on outdated national averages or general estimates. 

Donaldson Plastic Surgery in Columbus, Ohio, released real, anonymized implant inventory data from 12 months of procedures to help patients everywhere understand what aesthetic tastes really look like. Here’s what their patients actually chose and how the average size has changed over the years.

Nearly one in nine patients at Donaldson selected 485cc implants, making it the most common individual size of 2025. It sits in a sweet spot: enough volume to deliver noticeable enhancement and upper-pole fullness, while remaining proportionate for a wide range of body types. Think of a confident, curved silhouette that works in both a fitted dress and a swimsuit.

While 485cc topped the individual leaderboard, the 310 to 375cc range dominated overall, accounting for more than a third of all procedures. This is the “elevated version of yourself” zone. Patients choosing these sizes tend to want their results to feel like a natural extension of their existing shape rather than a dramatic departure from it.

The Average Breast Implant Size Is Getting Smaller

The average implant size across all 2025 procedures was 365cc, which aligns with a broader cultural shift. Over the past decade, smaller and lower-profile implants have become increasingly requested, particularly among patients who want to enhance their shape without signaling that they’ve had work done.

Donaldson providers call it the “ballerina” effect; refined proportions, subtle curves, and a result so seamless that even close friends might not notice. Their smallest reported range (180 to 240cc) accounted for 4.2% of procedures, which may look like a modest number on the surface, but was almost unheard of just five to 10 years ago. 

One in 13 Patients Used Two Different Sizes

About 7.97% of patients received implants of different sizes on each side. This is more common than most people realize, as human bodies are naturally asymmetrical. Using two different sizes is a technique that accounts for differences in natural breast volume, chest wall projection, or rib cage shape. It’s a sophisticated solution, an intentional decision, and it leads to more balanced results with profound patient satisfaction.

Every implant highlighted in Donaldson’s data is smooth silicone. Their surgeons favor silicone for its natural feel and lower risk of visible rippling. 

Smooth implants are the U.S. gold standard for good reason: they move naturally with the body throughout the day, maintaining a soft, unforced appearance. Textured implants, in contrast, adhere to surrounding tissue to stay fixed in place. Board-certified plastic surgeon and founder of the practice, Jeffrey Donaldson, MD, puts it plainly:

“The issue with the textured implant is that it has been implicated in a very rare form of lymphoma. I’ve always avoided textured implants in my practice because I just don’t even like the slightest chance of that.”

On the Other End of the Spectrum: 730 to 800cc

800cc is the maximum implant volume approved for cosmetic use in the U.S. At Donaldson, it accounted for 0% of 2025 procedures; not because they won’t use them, but because appropriate candidacy is rare. Dr. Donaldson has used implants in this range in carefully selected cases, but says it happens only a few times per year.

“Implant size is highly dependent on each individual’s preferences and anatomy. A great deal of conversation and exploration goes into finding the best implant. That’s why we allow patients to come back into our office as many times as they need to in an effort to find the best solution for them.”
