Home Health Easy Access To Care
In addition to the comfort and convenience of recovering at home, home health offers quick and easy access to medical care. Care is provided by a team of professionals who will help when an illness or accident affects daily life. Whether it is one hour or 24 hours, a full range of care is available seven days a week.
As per the doctor's order, a licensed home health agency representative will meet with the patient and/or family to perform a complete assessment of needs. An effective home care plan is then developed to meet those needs. Each case is evaluated on an individual basis from a clinical team viewpoint to ensure the most appropriate home care services are being provided.
Following the agency's intake process will ensure a quick and seamless transition of care from the hospital or doctor's office to the home. Some of the key elements in most intake processes include
The patient must be considered “homebound”; meaning there is a great and taxing effort to leave the home.
The doctor must give a written order for home health services to include the diagnosis and a completed face-to-face encounter (per Medicare guidelines) to initiate home care.
The patient's demographics must be submitted which includes the correct address, date of birth, contact number and insurance information.
Care will usually begin 24 to 48 hours from receipt of doctor's order with patient demographics.
No one should have to wait for needed medical treatment. Use home health and get the care you need now.
