Testing for Pyloric Stenosis in a Projectile-Vomiting Baby

Your baby finishes a feeding, seems comfortable for a moment, and then suddenly vomits with enough force to travel several inches—or farther. Was it ordinary spit-up, or could something be blocking the stomach? Pyloric stenosis is a narrowing at the outlet of the stomach caused by thickening of the pylorus muscle. It prevents breast milk or formula from passing normally into the small intestine. When Is Testing for Pyloric Stenosis Considered?
Many healthy babies spit up small amounts of milk without distress. Pyloric stenosis more often causes true vomiting that becomes increasingly forceful and frequent. It typically begins between about 2 and 8 weeks of age, although the timing can vary.
A clinician may suspect pyloric stenosis when a baby has several of these features:
- Forceful or projectile vomiting after some or most feedings
- Vomiting that becomes progressively worse over several days
- Vomit that looks like breast milk, formula, or curdled milk rather than green bile
- Strong hunger or an eagerness to feed soon after vomiting
- Fewer wet diapers, a dry mouth, reduced tears, or unusual sleepiness
- Poor weight gain or weight loss
- Fewer bowel movements because less food is reaching the intestines
- Visible wave-like movements across the upper abdomen after feeding

What Happens During a Pyloric Stenosis Evaluation?
Medical History and Physical Examination
The evaluation usually starts with detailed questions about the vomiting. The clinician may ask when it began, whether it follows every feeding, how forceful it is, and what color it appears. Information about feeding amounts, wet diapers, bowel movements, alertness, fever, and weight changes can help distinguish pyloric stenosis from other causes.
During the physical examination, the clinician checks the baby’s weight and looks for dehydration. The abdomen may be observed for contractions as the stomach tries to move milk through the narrowed outlet. An experienced examiner may sometimes feel the enlarged pylorus as a small, firm, olive-shaped mass in the upper abdomen. However, the mass cannot always be felt, so its absence does not rule out the condition.
Abdominal Ultrasound
An abdominal ultrasound is generally the preferred initial pyloric stenosis test for a young infant with new, non-green vomiting. Ultrasound uses sound waves rather than radiation to create images of the stomach and pylorus.
During the examination, a sonographer places gel and a small ultrasound device on the baby’s abdomen. The pylorus is viewed from different angles while the sonographer measures the thickness of its muscle and the length of its channel. The examination may also show whether fluid can pass from the stomach into the small intestine.
Healthcare facilities use established imaging criteria, but measurements must be interpreted in context. A baby’s size, age, positioning, stomach contents, and the quality of the images can affect the results. Parents should not try to diagnose the condition from a measurement in an imaging report without discussing it with the treating clinician.
An ultrasound is painless, although the baby may become fussy from hunger, positioning, or gentle pressure on the abdomen. Preparation instructions differ by facility. Parents should follow the imaging center’s directions about feeding before the appointment rather than withholding milk on their own.
What If the Ultrasound Is Unclear?
Pyloric stenosis can develop gradually. An early ultrasound may show measurements that are neither clearly normal nor clearly diagnostic. If symptoms strongly suggest the condition, the medical team may observe the baby, repeat the ultrasound after an appropriate interval, or use another imaging study.
An upper gastrointestinal, or upper GI, series may be considered when ultrasound findings are inconclusive or clinicians need to investigate another structural cause of vomiting. Because it uses radiation, an upper GI series is not usually the first pyloric stenosis test when a diagnostic ultrasound is available.
Blood Tests
Blood tests do not directly show whether the pylorus is thickened. Instead, they help determine how vomiting has affected the baby. The medical team may check electrolytes and other indicators of hydration and body chemistry.
Prolonged vomiting can lead to low chloride or potassium levels and an imbalance called metabolic alkalosis. These classic abnormalities may be absent when pyloric stenosis is recognized early, so normal blood results do not necessarily exclude it. If surgery is needed, dehydration and significant electrolyte abnormalities are generally corrected first with intravenous fluids.
What Happens After a Positive Test?
A confirmed diagnosis usually leads to evaluation by a pediatric surgeon. Treatment is a procedure called pyloromyotomy, in which the surgeon divides the thickened outer pylorus muscle so the stomach can empty more normally. The stomach and intestine themselves are not removed.
Pyloric stenosis is treated urgently, but surgery may not happen immediately upon arrival. Stabilizing the baby is the first priority. Feeding may be paused while intravenous fluids replace water and electrolytes lost through vomiting. Blood tests can be repeated to confirm that the baby is ready for anesthesia and surgery.

There is no reliable home pyloric stenosis test. Noting the timing, appearance, and frequency of vomiting can help the clinician, but testing requires a medical examination and usually imaging. When projectile vomiting is recurring, early evaluation is safer than waiting for obvious dehydration or weight loss to develop.
