What is Insulin Resistance? Does it Mean Prediabetes?

Insulin resistance means that muscle, fat, and liver cells respond less effectively to insulin than normal. The pancreas compensates by producing more insulin. However, insulin resistance does not necessarily mean you have prediabetes or diabetes.

Understanding the distinction between insulin resistance vs prediabetes matters because insulin resistance can appear before blood glucose reaches the prediabetes range. Recognizing the risk gives you more time to take early preventative action.

What Does Insulin Resistance Mean?

Insulin resistance is a condition in which the body’s cells respond less effectively to insulin. Insulin is a hormone secreted by the pancreas. Its main role is to help glucose move from the bloodstream into cells, where it is used for energy.

When cells become less sensitive to insulin, the pancreas usually compensates by producing more insulin. 

Insulin resistance often does not present with any obvious symptoms, so it may go undetected for years unless you proactively undergo an examination.

After you eat, carbohydrates get broken down into glucose and enter the bloodstream. Rising blood glucose signals the pancreas to secrete insulin.

Insulin then signals cells to take up glucose. Muscle cells can use glucose for energy or store some of it as glycogen. The liver also stores glucose and regulates how much glucose it releases into the bloodstream.

Does Insulin Resistance Mean Prediabetes?

No. Prediabetes and insulin resistance are not the same. Insulin resistance is a physiological mechanism, whereas prediabetes is a clinical diagnosis based on measurable blood sugar metrics.

  1. What are the Indicators for Diagnosing Prediabetes?

According to UK NICE guidance, people may be considered at high risk of developing type 2 diabetes based on blood glucose measures including: [3]

  • HbA1c: HbA1c result between 42 and 47 mmol/mol (6.0-6.4%) indicates prediabetes, while 48 mmol/mol (6.5%) or above falls within the diabetes range.
  • Fasting plasma glucose: 6.1-6.9 mmol/L
  • 2-hour plasma glucose during a 75-g oral glucose tolerance test: 7.8-11.0 mmol/L

Note: HbA1c reflects the average blood sugar level for the previous 2-3 months. Fasting plasma glucose measures blood glucose after fasting, while an oral glucose tolerance test evaluates how the body handles a glucose load.

Does Insulin Resistance Lead to Diabetes?

Insulin resistance can raise the risk of type 2 diabetes.

At first, the pancreas can often compensate by producing more insulin. This allows blood glucose to remain relatively stable despite reduced insulin sensitivity.

Over time, however, the pancreas may eventually fail to produce sufficient insulin to overcome the body’s insulin resistance. Blood glucose then starts to rise.

The first detectable stage may be prediabetes. If glucose regulation continues to deteriorate and reaches the diagnostic threshold, type 2 diabetes may be diagnosed. Insulin resistance and diabetes are closely related.

Not All Insulin Resistance Becomes Diabetes

Many people with insulin resistance do not have diabetes, and progression depends in part on how effectively pancreatic beta cells compensate by increasing insulin secretion.

What to Do After a Prediabetes Diagnosis?

  1. Can insulin resistance be reversed?  

Insulin sensitivity can often improve substantially, particularly when modifiable causes are addressed.
